It's still experimental, and may not do exactly what you want. I'd be interested in feedback. It is available on MathSource as "nb2tex". To access mathsource, send the email message "help intro" to mathsource at wri.com. Also accessible via ftp --- mathsource.wri.com. Send me email directly if you have trouble getting this.
