Regarding question (2), the code for LinearProgramming is in the
startup package LinearProgramming.m.  LinearProgramming constructs
a list of inequalities and calls ConstrainedMin.  ConstrainedMax also
calls ConstrainedMin after a trivial sign change.

ConstrainedMin uses a simplex algorithm with some straightforward
optimizations.
