I create my Mathematica graphics with minimal markup (axes labels, etc); export as EPS; import into Illustrator; hand insert or text, arrows, annotations, line weigths, other stuff in Illustrator; and export as Illustrator EPS. (And by the way I keep all my resulting illustrations in a master folder of illustrations, and use aliases to make them available to LaTeX or Textures.)
