One potential cause we've discovered in-house of a problem like this is 
when you're sharing your desktop with NetMeeting and you're running your 
display set to 16-bit color depth.  An easy workaround is to increase your 
color depth to 24 or 32-bit.  The problem could conceivably also occur 
under certain multiple monitor configurations, as well, although it 
wouldn't be very likely.  We have, of course, corrected the problem for 
future versions.

In any case, whether or not you are running NetMeeting or multiple 
monitors, I think changing your color depth and restarting Mathematica 
would be a good first thing to try.  Beyond that, it might be useful to 
know if anything has recently changed with your video hardware, drivers, 
or settings.  Let me know how it goes.
