The reason for the difference is bracketing.
Here are the main steps in the evaluations

#^2 & /@ (x /. x -> {a, b, c})
Map[#^2 &, x /. x -> {a, b, c})]
Map[#^2 &,  {a, b, c}]
{#^2 & [a], {#^2 & [a],{#^2 & [a]}
{a^2, b^2, c^2}


(#^2 & /@ x) /. x -> {a, b, c}
Map[#^2 & , x]/. x -> {a, b, c}
x/. x -> {a, b, c}
{a,b,c}
