Are you using a different input and output FormatTypes? Say StandardForm 
for input and TraditionalForm for output? I recall that this could cause 
such problems because the Notation package uses the FormatType to 
determine which form to create the notation in. It is possible to 
override this by adding the option WorkingForm->TraditionalForm when 
defining a notation.

Alternatively, try setting the menu entries 

  "Cell"->"Default Input FormatType" 

and 

  "Cell"->"Default Output FormatType" 

both to StandardForm (or TraditionalForm), quit the kernel, and then 
re-execute the Notebook.
