diff=Flatten[{list[[2]]-list[[1]],(Drop[list,2]-Drop[list,-2])/2,list[[-1]]-list[[-2]]}]

or

diff2=Flatten[Insert[(Rest[list]-Most[list])[[{1,-1}]],(Drop[list,2]-Drop[list,-2])/2,2]];
