Use double backslashes (as usual in Mathematica for Windows) or use ToFileName[]:

app = "D:\\Programme\\pythonika-1.0\\bin\\Windows\\Python2.4\\Pythonika.exe";
Install[app];


works for my installation of Pythonika.
