I can reliable reproduce what you're seeing if I evaluate

   <<Algebra`ExpressionManipulation`

(or the corresponding Get expression) more than once at a single kernel 
session.  However, I'm not getting any error messages at all if I use

   Needs["Algebra`ExpressionManipulation`"]

instead.
