Here is one way:

f[{y_,x_,z_}]:={x,y,2x+3y};
data={{1,2,0},{2,3,0},{4,1,0},{6,3,0},{1,3,0}};
TableForm[f /@ data,TableHeadings\[Rule]{None,{"x","y","F(x,y)"}}]
