Hi Alexei,

When you say "the heading appears only at the top of the
first page" is that because the cell is only placed at the top of the  
notebook and nowhere else? If you want the heading to appear at the  
top of every page you have to copy that cell to the top of each new  
page unless you place that content in a running head (aka, header).
