Two readers have contacted me with a work around:

For those who can't build the help browser index, go to your mathematica
directory\Documentation\English\MainBook, and rename browserindex.nb to something else.
It would be nice if someone with a working browserindex.nb would post it so that those with broken
index files can replace instead of renaming and losing a little from the complete help database.
(* Moderator's note: using someone elses
helpbrowser index file may or may not work, for
example my browser index file has references to
application packs installed after the original
Mathematica installation that may cause the
browser to stumble. *)

A working solution has been posted to the Mathgroup list. It's at
http://library.wolfram.com/mathgroup/archive/2002/Sep/msg00267.html
Actually in my case, I renamed BrowserIndex and BrowserIndex.nb in that
directory to BrowserIndex.bak and BrowserIndex.nb.bak. I then ran Mma 4.2
and entered the help browser, and everything worked correctly this time! I
exited Mma, renamed these files back to their previous names (BrowserIndex
and BrowserIndex.nb respectively) and everything still works fine. (At
least for now...haven't tried to rebuild the index.)
