I recently had a similar problem, where I had to reformat my hard drive and re-install Mathematica. What happens, rather annoyingly, is that a new MathID is generated when the program is re-installed and hence you need a new password. I had no trouble in filling in the online registration form, and receiving a new password from Wolfram. (Yes it's free - you own the software after all....)
