Kevin,
    Two things I can think of:
1.) Use Windows Explorer to check that UVW.m is found where you say it
should be. Make sure it is .m, not .nb.
2.)I notice that the package resides in drive e:, Is this really where the
rest of the Mathematica apps are stored?  In order to load the package, the
subdirectory containing it must be in the $Path for Mathematica.  Run the
$Path command to get a list of current paths.  If you don't find it there,
do

AppendTo[$Path,"e:\\math3\\AddOns\\Applications" ];

to complete the path list.
