Hi
modify the function f for simplicity
f[x_,y_]:={x,y,2x+3y}
and then
data={{1,2,0},{2,3,0},{4,1,0},{6,3,0},{1,3,0}}
and
myResult=(f@@Reverse[Most[#]])&/@data
generate the list and
TableForm[myResult,TableHeadings\[Rule]{None,{"x","y","f[x,y]"}},
TableAlignments\[Rule]Center]
generates the table
