This was a bug caused by quitting while the Help Browser is minimized.
To fix it, open the Help Browser and resize it to a non-zero size.  To
avoid it, either don't minimize the Help Browser, or make sure
you unminimize it before you quit Mathematica.

This should be a completely independent problem; I would be really
surprised if it were related to other problems you're having.
