What platform are you on? Your problem #1 sounds like the good old Mac OS 8 Appearance extension bug. (http://www.wolfram.com/support/Systems/Macintosh/FE/) I have never had any luck using PDFWriter with Mathematica. Try simply printing to a file and then process the file with Acrobat Distiller. Also, Distiller needs to be told where Mathematica hides its Type 1 fonts.
