I have no response from WRI so far.

I found a partial recovery solution at the following page:

I was able to retrieve healthy cells, but several text cells were
missing -- replaced with bad output from nearby input cells.  It's very
clear that when I evaluated the file, some outputs went where they
shouldn't have, and it occurred in several input cells at once.

I am running v4.1 on a Mac.  I have had the problem you describe 
below, but only when I have been foolish enough to rename a file that 
is open, by clicking on the icon and changing the name (in 
contradistinction to using "save as" "new name"). This seems to me to 
be a result of my operating system, but may give you a hint as to how 
it could occur with your system.
