Hi, 
thanks for your response.  Yes, unfortunately I have already checked both
locations to no avail.  The system would have overwritten any duplicate 
notebook copies in the same folder anyway.  I have the feeling that the problem
is multiple listings contained in an .ini file somewhere.

Jonathan,

Have you looked in Configuration>FrontEnd>Palettes as well as
SystemFiles>FrontEnd>Palettes?
