I don't know where it came from!  I do know I've been using it for a 
couple of years (well, the version that I created by immediately adding 
buttons to color text).

Oh, if only authors of Mathematica packages and palettes would embed 
some authorship information (name, affiliation/address, version number, 
date) in their files.
