Programmatic headers and footers

Using the File / Printing-Settings / Header-and-Footers you can set up 
headers and footers, but the scope for tailoring the results to your 
specialised requirements seems to be rather limited. Here are some examples 
that have defeated me. I am using Mathematica version 6.

1. How would you do an exceptional case if it fell on any page other than 
the first? It seems that I have to break up my document into small 
fragments, where each exceptional case lands on the first page of one of the 
document fragments, or something like that. Surely, there must be a better 
way?

2. How would you print page numbers as Roman numerals (e.g. as in document 
front matter)? I could do this if I had access to the kernel to do the 
translation, but I can't see how to do this kernel-based computation for 
headers and footers. Of course, I might not be looking in the right place in 
the documentation.

3. And, of course, the general case. How would you implement arbitrary 
programmatically generated headers/footers?
