I've had this happen when I've started up a system dialog (i.e., SystemDialogInput["Color"]) but then misplaced the dialog-window, perhaps because the dialog has been dragged nearly offscreen.  I believe that the "grayed out" is the intended behavior in this case, but it may be worth pointing out to others who see the original posting.
